双相障碍维持期治疗的新选择——钙通道调节剂与微量胺相关受体1激动剂的临床前景

A New Option for Maintenance Therapy of Bipolar Disorder——Clinical Prospects of Calcium Channel Modulators and Trace Amine-Associated Receptor 1 Agonists

  • 摘要: 双相障碍(bipolar disorder,BD)作为一种典型的精神障碍,其发病机制的确认、治疗药物的迭代以及治疗方式的更新均是值得关注的对象。BD维持期的治疗应将“防止复发”作为主要治疗目标,现有治疗药物和方案存在效果有限与副作用显著的局限性,需结合BD发病机制,寻找新的治疗靶点。围绕BD发病机制及治疗策略的新进展,重点综述Ca2+信号通路及微量胺相关受体1(trace amine-associated receptor 1,TAAR1)在BD发生发展中的作用,以及钙通道调节剂和TAAR1激动剂等新型治疗策略的研究进展,并分析两者潜在的协同治疗价值,旨在为BD新型治疗靶点的开发及精准治疗策略的优化提供参考。

     

    Abstract: Bipolar disorder(BD), as a typical mental disorder, has attracted increasing attention with respect to the elucidation of its pathogenesis, the development of therapeutic agents, and the optimization of treatment strategies. The primary objective of maintenance treatment for BD is the prevention of relapse. However, the currently available therapeutic drugs and treatment regimens are limited by unsatisfactory efficacy and significant adverse effects, highlighting the need to identify novel therapeutic targets based on the pathogenesis of BD. This review summarizes recent advances in the pathogenesis and therapeutic strategies of BD, with a particular focus on the roles of the Ca2+ signaling pathway and trace amine-associated receptor 1(TAAR1) in the occurrence and development of BD, as well as recent progress in novel therapeutic strategies, including calcium channel modulators and TAAR1 agonists. In addition, the potential synergistic therapeutic value of these two approaches is discussed, with the aim of providing a reference for the development of novel therapeutic targets and the optimization of precision therapeutic strategies for BD.
