急性缺血性脑卒中损伤机制及其治疗药物研究进展

Research Progress of Mechanisms and Drug Therapy for Acute Ischemic Stroke

  • 摘要: 急性缺血性脑卒中作为我国人口的重要致死、致残原因,已成为全球性严重社会负担。急性缺血性脑卒中的发生是由于脑动脉闭塞导致的脑组织缺血梗死,伴有神经元、星形胶质细胞及少突胶质细胞的激活后损伤,其病理生理机制主要涉及细胞能量衰竭、钙离子超载、兴奋毒性、氧自由基过度生成、炎症及凋亡等。针对缺血性脑卒中的临床诊治现状与可能的损伤机制,包括钙离子超载、氧化应激、凋亡自噬和炎症免疫相关信号,以及其治疗药物包括兴奋毒性靶向药物、抗氧化剂和抗硝基化损伤药物及抗炎制剂等作一概述,旨在为相关药物开发提供新的方向和理论依据。

     

    Abstract: Acute ischemic stroke remarkably intensifies disability and morbidity, leading to heavy global burdens. It is caused by cerebral artery occlusion-mediated ischemic injury, accompanied by activation of neurons, astrocytes and oligodendrocytes. Its pathophysiological mechanisms mainly involve cellular energy failure, calcium overload, excitotoxicity, excessive generation of oxygen free radicals, inflammation and apoptosis. This review is focused on the progresses of novel research on the current status of clinical practice and potential mechanisms, including calcium overload, oxidative stress, apoptosis, autophagy and inflammatory immune-related signaling pathways, with a discussion of such therapeutic drugs as anti-excitotoxic, antioxidating, anti-nitro damage and anti-inflammatory agents, aiming to provide new approaches and evidence for the development and clinical application of new drugs.
