Progress in Pharmacokinetics of Progesterone and Its Derivatives

  • 摘要: 黄体酮属孕激素类药物,对女性子宫内膜有显著的保护作用。黄体酮及其衍生物已被广泛用于子宫内膜增生、功能失调性子宫出血、子宫内膜异位症、经前期紧张综合征和黄体功能不全等各种妇科疾病。然而,部分黄体酮衍生物具有雄激素活性,可能具有致畸作用,在不孕症治疗中受到限制。黄体酮无雄激素活性,可以广泛用于辅助生殖领域,但治疗所需时间较长,黄体酮不同剂型的临床治疗效果差异较大,因此临床治疗取决于给药途径的选择。综述黄体酮及其衍生物的不同剂型的体内药代动力学参数以及给药途径、给药剂量和给药时间对其药代动力学的影响,以期为临床制定合理的给药方案提供参考。

     

    Abstract: Progesterone is a progestogen drug that has significant protective effect on the endometrium in women. Progesterone and its derivatives have been widely used in various gynecological diseases such as endometrial hyperplasia, dysfunctional uterine bleeding, endometriosis, premenstrual tension syndrome, corpus luteum insufficiency and so on. However, some progesterone derivatives have androgenic activity that may have teratogenic effects, and are limited in the treatment of infertility. Progesterone has no androgenic activity and can be widely used in the field of assisted reproduction. However, long-term treatment is required, and clinical efficacy of different progesterone dosage forms varies greatly. Therefore, clinical treatment largely depends on the choice of route of administration. This article reviews the in vivo pharmacokinetic parameters of progesterone and its derivatives in different dosage forms as well as the effect of the route of administration, dosage and dosing frequency on pharmacokinetics, so as to provide reference for the clinical formulation of a reasonable dosing regimen.
